What is a Patient-Centered Medical Home?
PCMH in its most basic form it means taking care of you is a team effort. Our health professional’s work together to keep you healthy and provide you with a new comprehensive approach to care. Our goal is to offer personalized attention with a focus on quality, satisfaction, and ways to lower costs. We have an active quality improvement team that meets quarterly and our patient advisory board meets bi-annually. Please let us know if you are interested in participating on the panel.

- 24/7/365 access to your medical records to view, download and or share with other providers platform at this time.
- Secure email allowing patients to receive results and messages. This is not a bidirectional platform at this time.
- Self-scheduling capability (limited to a portion of our providers schedules)
