Complete Physical Exams (CPE) and PAP's:
Your
PMG healthcare provider recommends a Physical Exam ("wellness exam")
every 2-5 years until the age of 50. After the age of 50, a CPE is
recommended annually. Insurance coverage varies for preventive care so
you may want to call your carrier first. In many cases, a "regular"
office visit charge will be covered by insurance even if a wellness
exam is not and this offsets some of the expense. Either way, wellness
visits are not expensive and they are critical to your long term
health. They are highly recommended.
When
scheduling for your CPE, please be aware that this is a routine visit.
Most busy practices typically book 3 months in advance for this
appointment. We are implementing a on a trial basis a new and hopefully
improved process for our Physical Exams-see the protocol below. Note
that if there are any medical questions or concerns that need to be addressed prior to your physical exam,
please schedule a regular office visit now and the Physical Exam
appointment can either be postponed or also scheduled but separately.
PAP/ Pelvic Exams are recommended annually for most women after age 18, sooner if sexually active. There are many insurance companies who do not pay for these services annually. Please call your insurance company prior to your visit to verify your coverage for this service. Even if a PAP smear is not covered, in many instances the exam can be a covered benefit.

PHYSICAL EXAM OFFICE PROTOCOL (TRIAL)
Thanks for your
patience as we start a new and improved way of completing physicals in our
office. Physicals will now be done in a 2 step process. The first step will be
a non-doctor visit to perform testing that your doctor has ordered. The second
step will be for the actual Complete Physical Exam with your doctor or
Physician Assistant.
Please download the
“Physical Examination Questionnaire" (below) and complete this form in its
entirety. Once completed, either mail or fax back to our office at 248-543-0562.
After the form is
received, we will review your Health Information Form and determine what type
of testing, if any, will be ordered. We will then call you to schedule a
non-doctor visit to perform any testing that has been ordered. Testing often
includes; fasting blood work, a Chest X-ray and/or an EKG.
Once your first
visit is complete, while leaving our office we will schedule you for the second
visit, which will be for your Complete Physical Exam. If you have any
additional questions regarding these steps, please don’t hesitate to call our
